ToggleEvery week I see patients who come to my consultation worried about their smile. Some bring photos they have saved for years, images of smiles they admire and quietly wish they could have.
Among those patients, a significant number share the same concern: when they smile, they feel that too much gum shows. I am Dra. Sara Pelaez Monsalve, cosmetic dentist at Clinica Viena in Medellin, and gummy smile treatment is something I perform regularly.
In this post I want to explain clearly what causes a gummy smile, which treatments actually work, how we decide the right approach for each person, and what you can expect if you come to us for this.
The first thing I always clarify is that a gummy smile is not a single condition with a single solution. The amount of gum that shows when you smile depends on several anatomical factors, and the correct treatment depends entirely on which factor is the primary driver in your case.
Getting this diagnosis right is what separates a result that looks natural from one that looks off. I have seen patients who received the wrong treatment elsewhere and came to us to correct it, so I want to be thorough here.

Gummy Smile Treatment Options at a Glance
Which approach is right depends on what is causing the excess gum display. Here are the options we use most.
Botulinum toxin
TEMPORARY RESULTInjected into the upper lip elevator muscles to limit how high the lip rises when smiling. Results last 4 to 6 months. Ideal for mild cases or as a first step to preview the corrected smile before surgery.
Crown lengthening
PERMANENTA periodontal procedure that removes excess gum tissue and, when needed, underlying bone to expose more of the tooth crown. The most commonly indicated treatment when teeth are truly covered by gum.
Lip repositioning
PERMANENTA minor surgical procedure that limits the upward movement of the upper lip by repositioning the lip-to-gum attachment. No bone removal. Effective when the gummy smile is caused primarily by lip hypermobility.
Smile design with veneers
PERMANENTWhen the teeth are the right length but the proportions still look off, porcelain veneers can restore proper width-to-height ratios. Often combined with crown lengthening for patients seeking a complete smile transformation.
Orthodontics
COMBINED TREATMENTIn cases of vertical maxillary excess (upper jaw protrusion), orthodontics or orthognathic surgery addresses the skeletal cause. Less common, but the only definitive solution for skeletal gummy smiles.
What Causes a Gummy Smile and Why It Matters for Treatment
Before I explain each treatment, I need to explain why the cause matters so much. A gummy smile is not one problem; it is the visible result of one or more underlying anatomical conditions.
The most common causes are: excess gum tissue covering the crowns of the teeth (a condition called altered passive eruption), a hyperactive or short upper lip that lifts too high when smiling, and vertical maxillary excess, which is when the upper jaw is positioned lower than ideal. In some patients, more than one factor is present at the same time.
This matters because each cause requires a completely different treatment. Crown lengthening solves excess gum tissue. Lip repositioning surgery addresses a hyperactive lip. Orthodontics or jaw surgery is the only definitive answer for skeletal causes. Botulinum toxin can reduce the appearance of a gummy smile from any cause, but it does so by limiting muscle movement rather than correcting the underlying anatomy. It is a functional mask, not a correction.
When a patient comes to me with a gummy smile, the first thing I do is examine the relationship between the lip, the teeth, and the gum line at rest and during a full smile. I measure how much gum shows and assess whether the teeth look short because gum is covering them or because the teeth themselves are small.
I look at the shape of the upper lip and whether it rises more than average. I also examine the bite. Only after this assessment can I tell a patient which treatment will actually solve their specific problem.
Botulinum Toxin for Gummy Smile: When and How We Use It
Botulinum toxin for gummy smile works by reducing the activity of the levator labii superioris, the muscle responsible for lifting the upper lip when you smile. When this muscle contracts too forcefully or has too much range of movement, it pulls the lip up higher than average, revealing more gum tissue than most people consider aesthetically ideal.
An injection of a small, carefully calculated dose of botulinum toxin into this muscle temporarily reduces its pull, so the lip does not rise as high during a smile.
The effect is visible within three to five days and lasts approximately four to six months. The procedure itself takes about ten minutes and requires no anesthesia and no recovery time.
For many of my patients who are not ready to commit to a permanent surgical procedure, or who want to see what their smile could look like with less gum showing before deciding on surgery, this is the ideal starting point.
I use it frequently as a diagnostic tool as much as a treatment: if a patient is happy with how their smile looks after botulinum toxin, it tells us a great deal about what a permanent result should look like and confirms that the hyperactive lip is the primary issue.
The limitation is that it is temporary. Patients who choose botulinum toxin as their long-term solution need to return every four to six months to maintain the result. Over time, some patients choose to transition to a permanent option once they are confident in what the corrected smile looks and feels like.
The injection also works best when lip hypermobility is the dominant cause of the gummy smile. When excess gum tissue is the primary issue, botulinum toxin reduces the visual impact of the gummy smile but does not address the gum itself, so the improvement is less dramatic.
Crown Lengthening: The Most Common Permanent Solution
Crown lengthening is the procedure I recommend most often for gummy smiles caused by excess gum tissue covering the tooth crowns. It is a periodontal procedure, meaning it is performed on the gums and the underlying bone, and its goal is to expose more of the natural tooth structure by removing the excess gum that is covering it.
The procedure involves numbing the area with local anesthesia, then carefully sculpting the gum line to the ideal position. In many cases, a small amount of underlying bone also needs to be recontoured to ensure the gum line stays stable in its new position. If we only remove gum tissue without addressing the bone, the gum can migrate back over time.
The healing period is typically three to five days of mild discomfort and some swelling, after which patients return to normal activity. Full healing and the final aesthetic result take about three months, at which point we can evaluate whether any further work is needed.
Crown lengthening is also the foundation for many smile design cases. When a patient wants porcelain veneers but their teeth look short because gum is covering part of the crown, we first perform crown lengthening to reveal the full tooth length, allow the area to heal, and then proceed with the veneers. Combining the two procedures gives results that look proportionate and natural, because we are working with the correct tooth dimensions from the start.
Lip Repositioning Surgery for Gummy Smile
Lip repositioning surgery addresses gummy smiles caused by a hyperactive or short upper lip. In this procedure, we detach a strip of the mucosa (the tissue that connects the inner lip to the gum), remove a small segment, and reattach the lip at a lower position. The result is that the lip cannot rise as high as it did before, because the range of motion is now physically limited. The correction is permanent.
The procedure is done under local anesthesia in about forty-five minutes to an hour. There is some swelling in the first week, and patients are on a soft diet for a few days. The sutures dissolve on their own, and most patients feel comfortable returning to normal life within five to seven days. The final result is fully visible after two to three months, once the tissues have settled into their new position.
Lip repositioning is often the procedure I recommend when a patient has a normal amount of gum tissue (meaning crown lengthening would not help) but their lip rises too high when they smile. We sometimes combine it with crown lengthening when both factors are contributing to the gummy smile, which is more common than most people realize. A thorough clinical examination before committing to any procedure is essential to identify all the contributing factors in each individual case.
Coming to Clinica Viena from the United States for Gummy Smile Treatment
A significant portion of our gummy smile patients travel from the United States, Canada, and Europe specifically for this procedure. The reasons are straightforward: the cost of crown lengthening and lip repositioning surgery in the US is often three to five times what the same procedure costs in Medellin, and the quality of care at Clinica Viena is comparable to the best private clinics in any major US city.
Our team speaks English, our facility is modern, and our patient coordination team handles scheduling from the initial consultation through the follow-up visit.
For patients traveling for gummy smile treatment, the typical visit is three to seven days depending on the procedure. Botulinum toxin requires only one appointment and no recovery time, so patients can be treated and continue their trip. Crown lengthening and lip repositioning require a short healing period, so we recommend staying in Medellin for at least five days after the procedure.
We coordinate with patients in advance, hold a virtual consultation before their trip, and prepare a detailed treatment plan so that when they arrive, everything is already planned and the first appointment can be the procedure itself.
Medellin is also a genuinely pleasant city to spend a week in while recovering. The climate is mild, the food is excellent, and the city has transformed dramatically over the past decade into a destination that many of our patients describe as a surprise. Several of our patients have told us that the trip itself was worthwhile entirely apart from the dental treatment.

Frequently Asked Questions (FAQ)
Is a gummy smile a medical problem or just cosmetic?
A gummy smile is primarily a cosmetic concern, but in some cases it is related to altered passive eruption, a condition where excess gum tissue covers part of the tooth crown. This can affect how the teeth look and how they respond to cleaning over time. In most cases, patients come to us because they feel self-conscious about their smile. Whether the motivation is cosmetic or functional, the treatments are the same and the results are permanent.
How do I know which gummy smile treatment I need?
The right treatment depends entirely on the cause. If your lip rises too high when you smile, lip repositioning is usually the answer. If your teeth look short because gum is covering them, crown lengthening is the starting point. If both factors are present, we combine the two procedures. The only way to know for certain is a clinical evaluation, where we measure the gum display, assess the lip movement, and examine the bone level. We also offer virtual consultations for patients traveling from abroad.
How much does gummy smile treatment cost compared to the United States?
The cost of crown lengthening and lip repositioning in the United States typically ranges from two to four times what the same procedure costs at Clinica Viena in Medellin. Many of our patients from the US and Canada find that even with the cost of travel and accommodation, the total investment is significantly lower than having the procedure done at home, and the quality of care is comparable to the best private clinics in any major city.
Is gummy smile treatment painful?
All our gummy smile procedures are performed under local anesthesia, so you feel no pain during the procedure. Afterward, crown lengthening and lip repositioning produce mild to moderate discomfort for three to five days, which is managed with over-the-counter pain relief in most cases. Botulinum toxin injections cause minimal discomfort and require no recovery time at all. Most patients are surprised by how manageable the recovery is.
Can a gummy smile come back after treatment?
Lip repositioning is a permanent procedure. Once the lip is repositioned, it cannot rise as high as before because the range of motion is physically limited by the new tissue attachment. Crown lengthening is also permanent when performed correctly, including the bone recontouring that stabilizes the new gum line. Botulinum toxin is the only temporary option and needs to be repeated every four to six months. Relapse after crown lengthening or lip repositioning is rare when the procedure is done properly.
How long do I need to stay in Medellin for gummy smile treatment?
For botulinum toxin, one appointment is enough and there is no recovery time, so you can be treated and continue your trip the same day. For crown lengthening or lip repositioning, we recommend staying at least five days after the procedure so we can see you for a healing check before you fly home. Most patients plan a stay of five to seven days total. We handle the coordination in advance so that when you arrive, everything is scheduled and your first appointment can be the procedure itself.
